- Idiopathic thrombocytopenic purpura (ITP) is the condition of having a low platelet count (thrombocytopenia) of no known cause (idiopathic).[1] As most causes appear to be related to antibodies against platelets, ITP is also known as immune thrombocytopenic purpura or immune-mediated thrombocytopenic purpura. Often ITP is asymptomatic, however a very low platelet count can lead to visible symptoms, such as purpura (bruises), or more seriously, bleeding diathesis.
Epidemiology
A normal platelet count is considered to be in the range of 150,000–450,000 per cubic millimeter (mm3) of blood for most healthy individuals. Hence one may be considered thrombocytopenic below that range, although the threshold for a diagnosis of ITP is not tied to any specific number. The incidence of ITP is estimated at 50–100 new cases per million per year, with children accounting for half of that amount. At least 70 percent of childhood cases will end up in remission within six months, even without treatment.[2][3][4] Moreover, a third of the remaining chronic cases will usually remit during follow-up observation, and another third will end up with only mild thrombocytopenia (defined as a platelet count above 50,000).[2] ITP is usually chronic in adults[5] and the probability of durable remission is 20–40 percent.[6] The male to female ratio in the adult group varies from 1:1.2 to 1.7 in most age ranges (childhood cases are roughly equal for both genders) and the median age of adults at the diagnosis is 56–60.[7] The ratio between male and female adult cases tends to widen with age. In the USA, the adult chronic population is thought to be approximately 60,000—with women outnumbering men approximately 2 to 1, which has resulted in ITP being designated an orphan disease.[8] The mortality rate of chronic ITP patients varies but increases as a function of age. In a study conducted in the Great Britain, it was noted that ITP patients experience an approximately 60 percent higher rate of mortality than gender- and age-matched comparison subjects without ITP. This increased risk of death with ITP is largely concentrated in middle-aged and elderly patients. Ninety-six percent of reported ITP-related deaths were patients 45 years or older. No significant difference was noted in the rate of survival between males and females.[9] ITP was first described by the German physician Paul Gottlieb Werlhof in 1735,[10] and was originally referred to as Werlhof’s disease.[11] The first report of a successful therapy for ITP was in 1916, when a young Polish medical student, Paul Kaznelson, described a female patient's response to a splenectomy.[12] Splenectomy remained a first-line remedy until the introduction of steroid therapy in the 1950s. Refractory ITP is a term used in chronic cases where thrombocytopenia persists despite the use of all clinically indicated therapies.
Signs and symptoms
Symptoms of ITP include the development of bruises (purpura) and petechiae, especially on the extremities, bleeding from the nostrils, bleeding at the gums, and menorrhagia, any of which may occur if the platelet count is below 20,000 per mm3.[13] A very low count (<10,000 per mm3) may result in the formation of hematomas in the mouth or on other mucous membranes. Serious and possibly fatal complications due to an extremely low count (<5,000 per mm3) may include subarachnoid or intracerebral hemorrhage, lower gastrointestinal bleeding or other internal bleeding. An ITP patient with an extremely low count is also vulnerable to major internal bleeding caused by abdominal trauma, as might be experienced in a motor vehicle crash. Fortunately, these complications are not likely in a patient whose platelet count is above 20,000. Well after 3 years, of hour long nose bleeds (some up to 6 hours) and heavy brusing, and lately my mouth being sealed shut from blood at night, i Got a blood test and was set to the ER, Everthings fine for the most part. My red blood Cells are normal, and so are my white. BUT my platelettes are deathly low, i have 4000, average human has 150,000. So my blood doesnt clot, and they say they are glad i came in, if i tripped and hit my head, i would have a HUGE possiblilty of dieing to blood going into my skull and not stopping. So right now im getting Platelette Fusions. So my swap is going to not be happening for either longer, bad luck on my part. all i can think about is getting out of here and mounting my RS spoiler i just had freshly painted. thats pretty bad haha. Well im on 20mg steriods 5 pills total. Im going to be having another blood test in the morning, Remember i was in the higher 5,000's now so i need that to go up to 10,000 before they will let me leave. so lets hope =( | |

Well i got a blood test at 6am today, just got the results back! platelets take awhile to grow and well after 5000, yesterday the tests resulted to 9000! so the biggest jump was today, SO FAR now it seems like the steriods may be able to treat me, but im still way to low, i may get out of here today and have blood treatments for weeks. hopefully with this not coming back. i really hope so.

| Craziness.... I wish you all the best and a speedy recovery. After your platelets are back up to more normal levels will they continue to recover or will this be something that you have to fight with for some time? I guess taking a med for life is better than the alternative. | |

| thanks guys and yes mike that will help, well hopefully left hand turns, since we all love the lewistown track the opposite way now!
the meds i will have to take for some weeks they said, and it should stabalize, if not they will see if somethings causing this. could be the spleen they said in worse case senerio, or bone morrow (highly doubt, really in 65+ age)
they said i should be fine after a few weeks on medication, they said most of the time, i will not need medication again, but it may just happen later down the road, they highly doubt that i will need to be on meds for awhile. | |

| The hospital will set you up on an interest free payment program but you have to kinda babysit them. Cause they lose track and send paperwork twice, dont file paperwork, send payments to the wrong department, lose payments etc. but as far as the plan itself they are pretty good about it. But all that is small potatoes if you are really sick. Good luck man. | |

| well had a blood test at 5:15am this morning, yesterday my count hit 11,000 in the morning, today the test showed i hit 13,000, so no HUGE jumps but it is going up, They feel im safe enough to leave but will require weekly blood tests for awhile. so finally i am out of here for good (hopefully)

| Thanks guys, we will see the Results weds to see if ill have a worse issue or what not. i really hope i got the Acute version (which i doubt, Acute will go away) . I really hate these Steriods they have me on, I feel pregnant lol. Working on my car isnt going well at all on these meds. Messed up my trunk and maybe my new spoiler. we will see.

| Will do Well got a Update, had a blood test today, still no results. its looking good though. Sadly ive gained 20 pounds in 5 days, because of the pills, got a moon face and belly, and the Acne is hitting hard now. ugh i look like a pudgy teen =( but aside from that still dont know the bill from the hospital yet, the day was going fine before... I accidentally rear ended someone today as well. woot! hoods crumpled and stuff. 500$ deductible, but the insurance is going to drop me now because of a previous ticket, even though this is my first accident ever. there goes 60$ full coverage, im just going to get my bills stable, pay off my car, park my car and buy a POS for the winter and ride my bike to and from work to get this belly fat off. Wish me luck haha, this is a horrible month for me. heres some fun pictures. | |

| Got called back on the Results today..
GREAT NEWS the doctors were shocked when they say the results, i hit 191k platelets, a HUGE jump from where it was.
They are Keeping me on the Steriods for one more setting just to be sure. i feel great. lets hope it stays =D | |

| That is wonderful you are feeling better Corey. I actually have a friend that has a stomach condition and when he takes his pills he has the same side effects. Everyone at work called him 'FACE'. That is fantastic things are starting to go in the right direction.
Sorry about to hear about the car. Been in that boat before and I must say I hope I never have to deal with it again.
